There are a num\u00adber of organ\u00adiz\u00ada\u00adtions in the US<\/span> try\u00ading to encour\u00adage the use of vari\u00adous stand\u00adards in health\u00adcare, but\u00adtressed by US<\/span> Fed\u00ader\u00adal funds. Here\u2019s a brief sum\u00admary of some of them, which glosses over many details. All of this is sub\u00adject to change at any time depend\u00ading on both fed\u00ader\u00adal and state polit\u00adics (in the US<\/span>, some parts of the health\u00adcare sys\u00adtem are fun\u00added and man\u00adaged fed\u00ader\u00adally, oth\u00aders at a state level). I wrote this for a friend in Ger\u00admany, and then thought oth\u00aders might also find it useful. <\/p>\n

Office of the Nation\u00adal Coordin\u00adat\u00ador for Health Inform\u00ada\u00adtion Tech\u00adno\u00adlogy (ONC<\/span>; http:\/\/healthit.hhs.gov\/<\/a>):<\/strong> part of the US<\/span> Depart\u00adment of Health and Human Ser\u00advices, the Nation\u00adal Coordin\u00adat\u00ador has the job of encour\u00adaging the adop\u00adtion of health\u00adcare IT<\/span> and appro\u00adpri\u00adate stand\u00adards. It\u2019s a rel\u00adat\u00adively new depart\u00adment, foun\u00added in 2004. It cer\u00adti\u00adfies health IT<\/span> products accord\u00ading to the stand\u00adards it adopts, and provides fund\u00ading for vari\u00adous aspects of health\u00adcare IT<\/span>, for example as part of Mean\u00ading\u00adful Use.<\/p>\n

Cen\u00adters for Medi\u00adcare and Medi\u00adcaid Ser\u00advices (CMS<\/span>; https:\/\/www.cms.gov\/<\/a>):<\/strong> runs Medi\u00adcare (health insur\u00adance for the eld\u00aderly as well as young\u00ader people with dis\u00adab\u00adil\u00adit\u00adies), Medi\u00adcaid (means-tested health insur\u00adance for those on low incomes), and the Chil\u00addren\u2019s Health Insur\u00adance Pro\u00adgram. CMS<\/span> does\u00adn\u2019t set the stand\u00adards for the clin\u00adic\u00adal care itself, but they do set stand\u00adards for report\u00ading the qual\u00adity met\u00adrics for clin\u00adic\u00adal care, as well as deal\u00ading with pay\u00adment for their programs.<\/p>\n

Mean\u00ading\u00adful Use (http:\/\/www.healthit.gov\/policy-researchers-implementers\/meaningful-use<\/a>):<\/strong> the US<\/span> ver\u00adsion of Mean\u00ading\u00adful Use is a health IT<\/span> pro\u00adgram that\u2019s meant to pro\u00admote the spread of elec\u00adtron\u00adic health records to improve health care in the United States. There are a num\u00adber of aspects to this pro\u00adgram. CMS<\/span> provides fin\u00adan\u00adcial incent\u00adives to phys\u00adi\u00adcians and oth\u00ader eli\u00adgible pro\u00adfes\u00adsion\u00adals, as well as hos\u00adpit\u00adals, to use cer\u00adti\u00adfied elec\u00adtron\u00adic health record (EHR<\/span>) sys\u00adtems, for example. The oth\u00ader part of the pro\u00adgram is decid\u00ading which stand\u00adards are the appro\u00adpri\u00adate ones for the EHR<\/span> sys\u00adtems to use. This is where ONC<\/span> plays a role. The pro\u00adject is being rolled out in stages across a num\u00adber of years. Stage 1 is also called the 2011 edi\u00adtion. Stage 2 was just released; the Stage 2 Mean\u00ading\u00adful Use set of stand\u00adards is also known as the 2014 Edi\u00adtion S&<\/span>CC<\/span> final rule and com\u00adple\u00adments the CMS<\/span> Stage 2 of the Medi\u00adcare and Medi\u00adcaid Elec\u00adtron\u00adic Health Record (EHR<\/span>) Incent\u00adive Pro\u00adgrams. There are con\u00adcrete plans up to Stage 3; any fur\u00adther stages depend on polit\u00adics and per\u00adceived needs.<\/p>\n

Health Level 7 Inter\u00adna\u00adtion\u00adal (HL7<\/span>; http:\/\/www.hl7.org<\/a>):<\/strong> an ANSI-accred\u00adited stand\u00adards devel\u00adop\u00ading organ\u00adiz\u00ada\u00adtion that has mem\u00adbers and affil\u00adi\u00adates in many coun\u00adtries. HL7<\/span> cre\u00adates health\u00adcare IT<\/span> stand\u00adards based on an over\u00adall archi\u00adtec\u00adture. Stand\u00adards can be norm\u00adat\u00adive or inform\u00ada\u00adtion, and go through a peri\u00adod of tri\u00adal use (Draft Stand\u00adard for Tri\u00adal Use; DSTU<\/span>) before becom\u00ading full stand\u00adards. The list of stand\u00adards in the DSTU<\/span> phase is at http:\/\/www.hl7.org\/dstucomments\/. The list of ANSI-approved HL7<\/span> stand\u00adards is at http:\/\/www.hl7.org\/implement\/standards\/ansiapproved.cfm, and the list of ISO-approved HL7<\/span> stand\u00adards is at http:\/\/www.iso.org\/iso\/search.htm?qt=HL7<\/span>&<\/span>sort=rel&<\/span>type=simple&<\/span>published=on. People who are not mem\u00adbers of HL7<\/span> do have to pay for the stand\u00adards, but HL7<\/span> has com\u00admit\u00adted to mak\u00ading them free of charge in early 2013. Sev\u00ader\u00adal HL7<\/span> stand\u00adards are quoted in the mean\u00ading\u00adful use reg\u00adu\u00adla\u00adtions that have been pub\u00adlished so far, both Stage 1 and Stage 2.<\/p>\n

Integ\u00adrat\u00ading the Health\u00adcare Enter\u00adprise (IHE<\/span>; http:\/\/www.ihe.net\/<\/a>)<\/strong> is an ini\u00adti\u00adat\u00adive to cre\u00adate pro\u00adfiles of vari\u00adous stand\u00adards to improve the way health\u00adcare IT<\/span> sys\u00adtems share inform\u00ada\u00adtion. They are an inter\u00adna\u00adtion\u00adal organ\u00adiz\u00ada\u00adtion with many mem\u00adbers and run con\u00adnecta\u00adthons to test inter\u00adop\u00ader\u00adab\u00adil\u00adity of health\u00adcare IT<\/span> sys\u00adtems both in the US<\/span> and in Europe. Many IHE<\/span> pro\u00adfiles and tech\u00adnic\u00adal frame\u00adworks are cited for Mean\u00ading\u00adful Use. There is sig\u00adni\u00adfic\u00adant cross-mem\u00adber\u00adship between HL7<\/span> and IHE<\/span>.<\/p>\n
